This form is to be utilized on behalf of all OMHA players that reside within the COWHA Zone that are between U14-U16 that wish tryout for another AAA Zone. Players will also require a Waiver from their respective Home Centre. Please note each Waiver needs to be signed by our President and we require 36hrs notice in order to respond with the executed waiver to provided email address.
