2018-19 Coaching Surveys have been delivered to your Team Staff and are Due December 20th

Please note that this years Coaching Surveys are due no later than December 20th. These forms are available through your team manager and are available for both head coaches, assistant coaches, player development and general comments.

These surveys are to be completed by each player, and his/her parents, confidentially, and returned to the team manager, in order to preserve their confidentiality please return them in a sealed envelope.  

They can also be placed in the Mail Box Marked VP-Hockey located on Pad 2 of the Lindsay Recreation Complex. While the completion of these forms is not mandatory, they are intended to assist the Coach Selection Committee, in (a) evaluating the suitability of a head coach to return as head coach,


(b) to determine whether any assistant coach should return as an assistant coach, (c) where an assistant coach has applied for a head coach position, to assist in evaluating that assistant coach's suitability to be a head coach, and (d) to provide feedback to the COWHA. 

The names of those who submit surveys will never be communicated to any Team Official.  The surveys will only be read by the VP-Hockey, who will provide a synopsis to the Coaching Selection Committee.  

These surveys must be signed to be given any consideration.  Unsigned or anonymous surveys will be disregarded and destroyed.  All surveys will be destroyed after all Team Officials have been approved by the Board.

This year, we are also asking parents and players to complete the attached Development Survey, to assist us in determining best practices for the development of our players.  Please have the completed Development Survey returned with the Parent/Player Satisfaction Surveys.
