Formal Written Complaint Form

This form is to be used by COWHA members for the purpose of submitting a formal written complaint regarding the behaviour and/or actions of another COWHA member. Upon completing this form, your complaint will automatically be submitted for review by the VP Business and the Director of Hockey Administration. A copy of your complaint will be emailed to you at the address you provide below. You will be contacted within 72hrs of submitting this complaint.
