Please note that the 2019-20 Hockey Programming, Development Programming/Skills and Coaching/Staff Surveys are now available to be submitted. Please fill these out as soon as possible. We are looking for one for the Parent(s)/Guardian(s) and one per each Player.
We understand their might be situations whereby a split family exists and each Parent/Guardian will wish to submit their own Survey.
These surveys will require you to submit your actual name, a valid email and a valid phone number so that we may confirm the Survey is related to the Team too which it was submitted.
The Surveys are held in strict confidence and will be used to with the other relevant surveys to give a general rating of the Programming, Development Programming and a specific rating the related Coach(es) and or Staff. All written comments are then added anonymously to the related file(s).
