2023-24 COWHA Program/Dev./Coaching Survey

Please find the 2023-24 COWHA Coaching Survey for Parents of Players Registered. You must enter your name within the Survey for it to be given any consideration. Unsigned or anonymous surveys will be disregarded and destroyed. All surveys will be destroyed after all Team Officials have been approved by the Board. If your Son or Daughter wishes to submit their own form they may do so by entering their own name and valid email and telephone. (or a parents email and telephone number)

Survey Indetifier

These Surveys are to be completed by each player, and his/her parents, confidentially, and submitted through our CONFIDENTIAL online form process. While the completion of these online Surveys is not mandatory, they are intended to assist the Coach Selection Committee, in (a) evaluating the suitability of a head coach to return as head coach, (b) to determine whether any assistant coach should return as an assistant coach, (c) where an assistant coach has applied for a head coach position, to assist in evaluating that assistant coach's suitability to be a head coach, and (d) to provide feedback to the COWHA. The names of those who submit Surveys will never be communicated to any Team Official. of Development Personnel. The Surveys will only be read by the VP-Hockey, President and the Coaching Selection Committee.
