2024-25 COWHA Coaching Survey (Central Ontario Wolves AAA)

You MUST enter your name/email/phone for your results to be given any consideration. The survey is focused on the head coach and the environment and culture he is creating. If you wish to provide feedback about assistant coaches and trainers, please use the comment box at the end. Only the Coaching Selection Committee will see individual surveys. Only aggregate data of an entire team will be shared with coaches and the COWHA Board. Individual surveys and names WILL NOT be shared with coaches nor the Board. If players wish to submit their own survey they may do so by entering their own name/email/phone or a parent's.
